Physical therapy patient portal for exercise plans

A physical therapy portal is where each patient gets an exercise plan from their therapist and logs their sessions against it, so the therapist reads real progress at the next appointment.

Pattern: Tracking

Describe it to Pory

“Our physical therapy clinic wants to give each patient their home exercise plan and have them log what they did, and how much it hurt, between visits.”

How Pory sets it up

Pory gives every patient and physical therapist a sign-in of their own. Each patient sees only their own exercise plans. It covers exercise plans & progress logs. Exercise plans, exercises, exercise logs, visits, forms, statements and team live in Pory's own database, so there's no spreadsheet to keep in sync.

What it does

01

Every patient leaves with their exercise plan, not a photocopy

Home programs go out as a photocopied handout, and by week two the patient has lost it or is guessing at the reps. The therapist keeps one plan per patient, with each exercise, its sets and reps and how often, and the patient's own log entries on the same record. Changing a rep count means editing one line, not printing a new sheet.

Patients log sets and a pain score between appointments

Between appointments the therapist only hears "yeah, I've been doing them, mostly." Now the patient logs each session from their phone: the date, which plan, what they managed and a pain score out of ten. At the next appointment the therapist opens the plan and reads the week, instead of relying on two people's memories.

The parts behind this portal

Each one reads and writes the same records, so a form, its approval and the status page someone checks are one system. How Pory builds a portal

  • Forms

Signed-in or public, with typed fields and file uploads. A form files a record; it doesn't start an email thread.

  • Detail view with related items

Open a unit, a pet or a listing to see its details and everything tied to it: requests, visits, offers. Share it by link; edit, change the status or add a related item from there.

Control

3 parts
  • Who sees which records

Each person sees their own records, the ones assigned to them, or the ones they manage. Fields can be hidden per role. Pory sets this up from your description, and you can adjust it in the editor.

  • Passwordless sign-in

A sign-in link by email, by default. Password sign-in if you'd rather.

  • Your brand

Your domain, your colors, your logo. Members see you.

Frequently asked questions

A patient signs in with a link sent to their email and lands on Home. They see Home, My exercise logs, My plan, Visits, Forms and Statements, and only the exercise plans that are theirs. From there they can use “New exercise log” and “Upload a document”.

You invite them. Each person gets a sign-in link by email, with no password to set, and lands in the role you chose for them.
